As an HIV/AIDS casemanager I have met many clients with unique situations, but none as unique as this one. This one has breast cancer along with a diagnosis of HIV. Based upon a preliminary review of the literature, evidence appear to be advancing the idea that hormonal therapy is indicated as opposed to radiation and chemotherapy. However, the client’s oncologist is anxious to treat the malignacies, that have since mastaticized to her back, with the contraindicated approach, radiation and chemotherapy. How do I express my concern to the client, without causing alarm or interfering with medical judgement. Radiation and chemotherapy are knownn to decrease the CD4 count and to suppress it below pre-therapy levels. My client’s CD4 count is 400 and I am concerned that the radiation and chemotherapy will leave her immune system totally defenseless. If anyone out there has knowledge regarding the comorbidity of these two diseases, please respond to.

A performed at the Centers for Disease Control Conference in Atlanta reveals that most HIV infected men go for unprotected sex only with others who are infected with HIV.

More on the plate than any one person should have to survive.....I can relate. Bipolar, Colon cancer survivor, HIV, Hep C, HPV.....Couldn’t be happier in life today just to be living and communicating with people.

Had a quick look to see what the literature was saying

Firstly , may be difficut to find expert opinion on this one as the number of cases of HIV and Ca Breast is low (In fact HIV seems to lower the risk of this cancer .1)

There seems to be a bit of evidence that a good immune system is needed to prevent 2.
("available data, however, suggest that antitumor response and survival in HIV-infected people with malignancy are improved in people with higher CD4 counts" 3.)

Have a look at the papers they might help, personally and from what I’ve been taught (and am being taught as I’m merely a token medical student the patient should be given all her options so she can choese. This may be difficult for the you to do, as you will have to take flack of both the patient’s grief .
It may also be difficult for the oncologist as not being able to cure is a difficult thing for medics to deal with (its why many of us go into it and not being able to cure is seen or feels like failure). Perhaps talk to the oncologist first (and other allied health professionals she is working with) one of these maybe more suitable to make the recommendations than you.
